There's a lot of information out there on how to unlock Act 3 and let me tell you that in my case, none of it helped, because despite completing every single wish I had stumbled upon in every settlement, exploring basically all the map for 60 hours and, rescuing almost all fleas, I still hadn't unlocked Act 3. So, what was I missing?

Before I jump into the final piece of the puzzle, let me get some facts straight on what you do NEED to complete.

For the Flea Caravan the most important thing is NOT just the number of fleas you need to find, but actually, meeting 3 key characters to make them join the caravan: First, the Huge Flea atop of the Memorium, the warrior flea Vog found in a secret cave on top of the Bellway in the Putrified Ducts and the spa manager flea Kratt found near the passage that leads to Sinner's Road. Besides those fleas, you also need to discover the big lake at the far east side of the Putrified Ducts. Once you have checked all of that and rescued enough fleas (in my case, 20), the flea caravan will move to its final destination, and Fleatopia will be born. I'll have a video later when I find the remaining ones I need to find, but in the meantime, I'd recommend you to check MapGenie's Silksong map to help you with this and any other missing things you need. The second important factor is for you to complete some wishes for each settlement. I am a completionist, and I had all of them done, but my guess, thanks to the dialogues that pop up later with the sidequest that triggers Act 3, is that you focus on every wish that helps each settlement get to the best shape possible. That includes getting the statue of Bone Bottom built, getting your Bellhome unlocked as gratitude for your donations at Bellhart, and helping Sherma with the supplies and donating to the camp at First Shrine. Those should be the most important ones you should focus on.

Finally, the obvious part is collecting the 3 melodies, which is the main objective in Act 2, and having everything ready for the final boss fight.

In case you've checked all of the items in this list and the quest has already triggered for you, well done. BUT, if you're still not getting the sidequest, here are 2 additional options for you to check:

The one I should prioritize would be to defeat Groal the great, the boss at the very top of Bile Water. This will give you access to a small hut where you'll find the Seeker's Soul item, which is in fact one of the 4 items needed in the sidequest that leads to Act 3.

If that doesn't work, complete finding Shakra in ALL possible locations and buying everything from her. This will trigger her sidequest, which is completed very near the boss fight against Groal the Great in Bilewater.
I did both of them before going back to the First shrine, and with this, the Caretaker was waving at me and the Side Quest was unlocked.

Now, moving to the sidequest, it's actually pretty simple. You need to get 4 items. The first is the Maiden's soul from the old lady back at the Ruined Chapel, easy peasy.
The second one is the Hermit's soul from the annoying guy found below Bellhart's bellway.
The third one is the one we already got after defeating Groal the Great in Bilewater.
And finally, you need a trap found inside a secret room in Weavenest Atla. Right here.

Once you have gather all those items, go back to the Caretaker, talk to him to deliver all the items and face the final boss at the very top of the Citadel. When you defeat the boss, select the option to play the Needolin and enjoy the ending cutscene and select your file again when you reach the start menu to start Act 3.
